Terms:
    1. Payments may be made by cash, cheque, electronic funds transfer, or debit order. All cheques must be made
        payable to Sporting Football Club. Our banking details are as follows:
        ABSA Bank Southdale, Branch Code: 507305, Account No: 2320171736.
    2. Payment must accompany this registration form.
    3. Players owing subs from a previous season will not be registered until the outstanding subs, as well as
        current subs have been paid in full.
    4. Registration / membership fees are not refundable.
    5. The club reserves the right to grant club membership at its discretion and in accordance with the club's
        constitution.
    6. All players are bound by the same payment terms, including those whose registration / membership fees
        have been sponsored.
    7. Any player whose registration / membership fees have been sponsored by the club or any of it's coaches or
        managers, hereby agrees that should he / she leave the club for any reason whatsoever during the course of or
        at the end of the year in which they were sponsored, then their full membership fees for that year would
        become due by them, and the player's clearance would not be issued until such due fees were paid in full.

Option 1 – Outright payment – R700.00 for 1st child / R500.00 for any child thereafter

I/We understand that the withdrawals hereby authorized will be processed by computer through a system provided
by the South African banks and I also understand that details of each withdrawal will be printed on my bank
statement.
I/We agree to pay any bank charges relating to this debit order instruction.

B. Mandate:

I/We acknowledge that all payment instructions issued by you shall be treated by my/our abovementioned bank as if
the instructions had been issued by me/us personally.

C. Cancellation:
I/We agree that although this authority and mandate may be canceled by me/us, such cancellation will not cancel my
obligations to Sporting Football Club in terms of any outstanding subs due to you. I/We also understand that I/we
can not reclaim amounts, which have been withdrawn from my/our account (paid) in terms of this authority and
mandate if such amounts were legally owing to you.

D. Assignment:
I/We acknowledge that the party hereby authorized to effect the drawing(s) against my/our account may not cede or
assign any of its rights to any third party without my/our prior written consent and that I/we may not delegate any of
my/our obligations in terms of this authority to any third party without prior written consent of the authorized party.

Indemnity:

    her behalf and pursuant to my child/ward's involvement and participation in Sporting Football Club
2. Hereby agree, acknowledge and undertake in favour of Sporting Football Club, it's management, coaching
    staff, employees, committee members, agents and representatives that I am fully aware of the unlimited risk
    occasioned by my child/ward's involvement in the club and accept all risks associated therewith.
3. I hereby release and hold harmless each and all of the coaching staff, employees, management, committee
    members, agents and representatives against all claims arising out of or in any way occasioned by my
    child/ward's participation in the Sporting Football Club activities, tours or excursions including without
    limitation:
    ● Any personal injury / loss of life
    ● Any loss or damages to personal clothing or property belonging to my child or any other third party
    ● Any loss of support or consequential loss
    ● Any medical expenses
    Whether arising out of strict liability, statute, negligence, or any other cause whatsoever.
4. I nominate as my proxy the team coach / manager, or in their absence their nominee, in the event of an injury
    which requires attention to take whatever steps necessary until I am able to be contacted and hereby accept
    that I will be responsible for the payment of any medical and / or hospital fees.
5. I also undertake that my child / children, members of my family and myself shall abide by the constitution
    and code of conduct of Sporting Football Club and respect the association's laws and bye-laws.
